** Current opportunities are located within the Brentwood, Smithtown, Islandia, E. Islip, Coram, and Port Jefferson Station communities. **
General Purpose of the Job Responsible for monitoring and assisting in the overall health and safety of the individual served.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Supervise and train direct care staff regarding medication administration, universal precautions, infection control, health and safety issues, etc.
Complete yearly recertification, including practicum, for each medication certified person.
Implement guidance policy as appropriate to address medication errors or violations of health and safety procedures by direct care staff.
Available by phone 24-hours to address medical emergencies.
Insure maintenance and review of all medical records per state and federal guidelines and UCP policy.
Meet regularly with team members to review current issues.
Insure all indicated and annual medical appointments and labwork are scheduled and followed-up appropriately and as per UCP policy.
Monitor and review medication administration record, recording of medications, medication orders, labels, etc. Insure an adequate supply of medication on hand.
Monitor and maintain an adequate supply of medical goods and equipment, as well as first aide supplies.
Complete all paperwork as assigned by director of Residential Services of designee, and according to UCP policy (to include monthly nursing notes, self-administration of medication evaluation, infection control reports, etc.).
Document medical and pharmacological services and issues. Insure pharmacy review of medication every 90 days.
Insure individuals are living in a safe and hygienic manner.
Give yearly recertification, including practicum, to each (AMAP) medication certified aide, and to certify staff in tube feeding, colostomy care and diabetes care as needed.
Other responsibilities as required.

Education and/or Experience
Associate's degree (A. A.) or equivalent from two-year college or technical school and one to three years related experience and/or training.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
