Position Overview
The Licensed Practical Nurse plays an important role in supporting public health services at SIPH. This position provides direct clinical care, patient education, and care coordination related to immunizations, family planning, and sexually transmitted infections services. Working collaboratively with providers, clinical teams, and community partners, the Licensed Practical Nurse helps ensure clients receive respectful, high-quality care in support of SIPH's mission to promote and protect the health of the communities we serve.
Schedule & Hours
The typical schedule is 40 hours per week, Monday through Thursday, 8 am to 6 pm.
Key Responsibilities
The Licensed Practical Nurse provides clinical, administrative, and community-based support services to advance public health programs and ensure high-quality, client-centered care.
Clinical Care
• Performs skilled nursing services based on SIPH policies, procedures, or protocols.
• Educates, counsels, and administers immunizations to client.
• Coordinates and conducts offsite immunization clinics.
• Educates and counsels client of STD's, testing, and treatment.
• Educates and counsels client on family planning methods, pregnancy prevention, and STD prevention.
• Documents client's vitals, findings, and assessments within the electronic medical record and signs encounter note within a timely manner.
• Performs and/or orders appropriate labs, testing, medication, and treatments as requested by the provider in an accurate and timely manner.
• Helps coordinate referrals to medical providers/community resources for clients as needed.
• Delivers care with appropriate age and cultural competence to specific client populations according to individualized needs.
• Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the client's diversity, preserving their autonomy, dignity, and rights.
• Initiates emergency protocols when deemed necessary and appropriate (i.e.; anaphylaxis response).
• Maintains confidentiality in matter related to the client, family, and clinical services staff.
• Maintain patient confidentiality and protect clinic operations by abiding with HIPAA and keeping patient care information confidential.
Administration
• Attends, engages, and provides feedback during Clinical Service Team meetings.
• Ensures any assigned contract/sub-grant deliverables are met.
• Actively participates in quality improvement.
• Monitors family planning, STD, and immunization inventory and communicates with CNA, Purchaser, and Clinical Services Director on discrepancies or needed items.
• Follows established Idaho Immunization Program policies and protocols for maintaining immunizations in fridge, freezer, and while in transit.
• Fosters and empowers a team-centric approach within CS and SIPH as a whole.
Community Resource
• Conducts outreach to community partners to regularly inform about services SIPH's provides.
• Serves and educates community partners on various related health topics.
• Conducts school visits, provide education, and evidenced based screenings.
• Other duties as assigned.
Minimum Qualifications
Requires current licensure as a Licensed Practical Nurse or state licensure as a Licensed Practical Nurse pursuant to the Nurse Licensure Compact, Idaho Code 54-1418.
