This is a part time position scheduled to work 3 consistent days per week.
GENERAL POSITION DESCRIPTION:
Provide comprehensive, unbiased nursing care across Delta County Health Department’s public health programs, with a focus on family planning services and support of communicable diseases and child health to ensure smooth clinic operations and high-quality community health services.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
• Health Assessment & Care: Conduct health assessments, gather histories, administer diagnostic tests, and provide nursing services such as vitals checks, immunizations, and medication administration.
• Program Management: As part of a larger strategic plan, build, maintain, conduct processes and outcome measures of supportive grant programs as assigned. Responsibility to adhere to grant reporting guidelines.
• Care Planning & Case Management: Develop individualized care plans based on client needs and goals, implement and re-evaluate these plans, and make referrals to other providers or resources as needed.
• Program-Specific Duties:
• Family Planning: Provide counseling, education, sexual and reproductive health services to individuals and families. Assess client needs, facilitate access to contraceptive methods, conduct STI screening and treatment and offer prenatal and postnatal care.
• Chronic Illness & Disease: Assesses and evaluates individuals for and affected by chronic illness and diseases. Plans, implements and monitors prevention strategies and coordinates treatment, education and follow up services related to public health programs.
• Immunizations: Be able to provide support to educate clients on immunizations, manage vaccines, maintain records in the state immunization system, and oversee off-site clinics.
• Communicable Diseases, Tuberculosis & Disease Investigation: Be able to conduct TB and communicable disease screenings, provide education, and manage treatment plans and perform epidemiological investigations in line with CDC and CDPHE guidelines. Educate the community on prevention measures, track cases, and maintain accurate records.
• Child Health: Perform care coordination, develop family care plans, and manage documentation for child health programs.
• Collaboration: Collaborate with team members to support overall clinic operations, ensuring efficient and effective care across all service areas.
• Health Promotion & Education: Offer counseling, education, and outreach to individuals, families, and communities on disease prevention, wellness, and available health programs.
• Documentation & Compliance: Maintain accurate records in compliance with health department policies, grant deliverables and programs including using electronic health records and following HIPAA standards.
• Collaboration & Professional Growth: Work effectively with other health department staff across diverse programs and participate in health department and community committees, staff meetings, and educational programs. Assist with program development and funding efforts.
• This position requires consistent punctuality and attendance. The primary location of employment for this position is the designated Delta County location/office with the exception of duties that are performed off site as required in responsibilities. The employee is required to perform most (generally, 95%) of assigned duties from this designated office location to allow for client, customer and coworker access and collaboration, unless prior written approval for an alternative arrangement, such as temporary remote work, has been granted by the employee's direct supervisor and/or department head and maintained in the employee personnel file.
GENERAL JOB DUTIES:
• Receives clinical oversight and guidance from the Nurse Practitioner.
• Work effectively with other health department staff across diverse programs and participate in health department and community committees, staff meetings, and educational programs. Assist with program development and funding efforts.
• Perform inventory management, facility preparation, and equipment sterilization.
• Participate in epidemiological investigations and conduct community health surveys.
• Market health department services and maintain professional relationships with community partners, CDPHE staff, and medical providers.
• Flexibility to perform related duties as assigned, support disaster preparedness, work well with diverse cultures, and contribute to a cooperative team environment.
• Demonstrate core competencies within the Incident Command System structure as appropriate to this position’s functional role in a public health emergency response event. (Completion of NIMS-700, ICS-100, ICS-200)
• Should the Board of County Commissioners or another authorized representative issue a local disaster declaration in Delta County, all Delta County employees may be called upon to serve as disaster service workers.
This position is grant-funded and contingent upon the availability of grant funds; continuation of employment is dependent on ongoing funding and the position may be eliminated if grant funding is reduced or discontinued.
Requirements
POSITION REQUIREMENTS:
Education:
Associate’s degree in nursing required, bachelor’s degree preferred
Experience:
• At least three (3) years of nursing experience in a Public Health setting, physician’s office, or community clinical setting is preferred.
• Proficiency and experience in AthenaOne (electronic health record) is preferred.
Combination of education and experience could waive specific requirement.
Licenses/Certifications:
• Current and unrestricted Colorado Registered Nurse license
• Current and unrestricted American Heart Association BLS certificate
• Current and valid Colorado Driver’s License
