Regional Registered Nurse (RN) – Patient Intake & Assessment in Melbourne, FL

We are seeking a detail-oriented and compassionate Regional Registered Nurse (RN) to join our nurse registry as an independent contractor. The RN will be responsible for performing initial patient intake assessments to help determine the level of care required when new referrals are received In Brevard County. This role is ideal for an experienced RN who enjoys working autonomously and has a strong clinical judgment for care planning in the home care setting. Primary Responsibilities: • Conduct timely in-home intake assessments for new patients • Evaluate the patient’s condition, living environment, and support system • Determine the appropriate level of care (personal care, homemaking, nursing services, etc.) • Complete all required documentation accurately and in accordance with state regulations • Communicate assessment findings to the care coordination team • Make recommendations for follow-up services, safety needs, or special considerations • Maintain professionalism and confidentiality at all times • Willing to Commute throughout Brevard to complete patient assessments Qualifications: • Current, RN license in the State of Florida • Minimum of 1 year of nursing experience (home health or case management preferred) • Strong assessment, documentation, and communication skills • Reliable transportation and willingness to travel within the region • Ability to work independently and manage time effectively • Must pass a background check Benefits of Joining Our Registry: • Flexible scheduling based on your availability • Independent contractor structure • Supportive and responsive coordination team • Meaningful work impacting the lives of clients and families Job Types: Part-time, PRN Pay: $40.00 per hour Work Location: In person
