Position Summary: This position is direct patient care and administrative in nature and is designed to meet the needs of individuals served by the ACT program in alignment with CCBHC standards. The Registered Nurse works as part of a multidisciplinary ACT team to provide nursing services, medication management, health education, and crisis intervention in community settings. The RN collaborates closely with psychiatric providers, case managers, and other team members to ensure coordinated, recovery-oriented care. The Assertive Community Treatment (ACT) program provides intensive, team-based, community-delivered services to adults with serious and persistent mental illness, with services available seven days per week. Duties/Responsibilities: • Registered Nurses work closely with supervising psychiatrists and nurse practitioners to assess and monitor client health and medication needs by: • Assisting with medication clinics and treatment planning • Monitoring medication effectiveness, side effects, and symptoms • Communicating clinical observations to prescribers and the ACT team • Administering medications and treatments per orders and protocols • Completing timely, accurate documentation in the electronic health record (EHR) and medication administration records (MARs) • Complying with federal, state, and local nursing regulations, standards, and organizational policies • Collaborating with internal staff and external partners, including pharmacies and medical providers • Completion of required continuing education and participation in annual performance review processes. • Adherence to organizational code of conduct and maintenance of appropriate professional boundaries with clients, staff, and community partners. • Other duties as assigned within the RN scope of practice to meet program and organizational needs. Program-Specific Duties / Responsibilities: • Provide community-based nursing services to adults enrolled in the ACT program. • Oversee medication administration systems in compliance with Minnesota regulations, including preparation, storage, distribution, and monitoring of medications. • Train ACT staff on medication administration procedures, side-effect monitoring, and best practices. • Provide individual and group medication education, including facilitation of a weekly medication education group. • Support self-medication programs as clinically appropriate. • Assist clients with medical appointments, coordination of care, and follow-up with external health providers. • Participate in crisis assessment and intervention, following established ACT and organizational procedures. • Respond to crisis situations immediately when on-site or within one hour when on-call. • Document all significant client contacts, nursing interventions, and crisis events in accordance with ACT, CCBHC, and regulatory requirements. • Attend ACT clinical, treatment team, and program meetings. • Maintain required documentation and participate in required trainings, including a minimum of 16 hours annually as specified by ACT/IRT/CS standards. Required Skills/Abilities: • Strong clinical assessment, judgment, and crisis management skills. • Excellent verbal and written communication skills. • Ability to work independently and collaboratively within a multidisciplinary team. • Non-judgmental, recovery-oriented approach to individuals with serious mental illness. • Strong time management, organization, and adaptability in a community-based environment. • Proficiency with electronic health records and basic computer applications. • Ability to maintain confidentiality and professional boundaries. • Regular and reliable attendance is a requirement to this position Education and Experience: • Current Registered Nurse license in the State of Minnesota (required). • CPR certification required. • Must be able to pass a Minnesota Department of Human Services background study. • Preference for experience in community mental health, ACT, or adult mental health services. • Preference for BSN-prepared nurses due to the community health component of the role. Physical Requirements: • 18 years of age. • Ability to sit, stand, bend, reach, and lift up to 50 pounds. • Ability to navigate community environments, including stairs. • Ability to physically perform nursing procedures, including injections. • Vision sufficient to utilize a computer monitor, monitor clients, and prepare and administer medications. • Hearing sufficiently to communicate in person and by telephone, individually and in groups. • Maintain acceptable standards of hygiene and professional appearance. • Must maintain a valid Minnesota driver’s license and auto insurance.
