Registered Nurse ACT Team - Brooklyn, NY (Onsite) - Contract M-F, 09:00am-05:00pm with One Hybrid Day - $50-55 Per Hr Title: Registered Nurse ACT Team NYC - 1 day hybrid We are seeking a compassionate and skilled Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. This role involves providing holistic, person-centered nursing care to individuals with serious mental illness (SMI) and co-occurring disorders in a community-based setting. The ideal candidate is passionate about working with underserved populations and thrives in a mobile, team-oriented environment. Location: Brooklyn, NY Employment Type: Contract M-F, 09:00am-05:00pm With One Hybrid Day Compensation: Salary: $50-55 Per HR Benefits: • Weekly pay • Guaranteed Hours • 401k retirement plan • Sick pay • Referral bonus • Medical benefits • Dental benefits • Vision benefits ESSENTIAL DUTIES & RESPONSIBILITIES: • Provide medication education, administration, and monitoring to ACT clients. • Conduct comprehensive nursing assessments, focusing on both physical and mental health needs. • Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans. • Support clients in managing chronic health conditions and accessing medical care. • Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction. • Participate in daily ACT team meetings and interdisciplinary care planning. • Respond to crises and provide on-call support as needed. • Ensure compliance with state and federal regulations, maintaining accurate clinical documentation. REQUIRED EDUCATION AND EXPERIENCE: • Licensed Registered RN within the state of New York. • Must have excellent verbal, written and interpersonal communication skills.
