Job Description
• Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance.
• Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care.
• Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions.
• Collect, assess, and evaluate patient health data to develop and implement care plans.
• Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.
• Utilize EMR and other systems to document care accurately and efficiently.
• Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency.

Qualifications
• Graduate of an accredited Nursing Program and valid NJ RN license required.
• BSN preferred.
• 3+ years of experience in an outpatient or ambulatory setting; 2+ years in acute care (preferably critical care).
• Strong communication, organization, and computer skills.BLS certification required (or obtained after hire).
