Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) – RN Preferred
Salary: $85,000–$95,000 annually
Location: Valley Vista for Nursing and Rehabilitation | Newton, IA
Status: Full-Time
Valley Vista for Nursing and Rehabilitation is seeking a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) to join our skilled nursing team in Newton, Iowa. This full-time leadership role offers the opportunity to make a meaningful impact by ensuring high-quality resident care through accurate assessments, regulatory compliance, and effective care planning.
Your Role & Responsibilities
As the RNAC / MDS Coordinator, you will:
• Conduct routine resident assessments per schedule and as needed for condition changes, hospital discharges, and other clinical events
• Accurately observe, assess, and communicate changes in resident condition to appropriate interdisciplinary team members
• Collaborate with Social Services, Activities, Dietary, and Nursing to ensure timely completion of assessments and CAAs with complete documentation
• Complete MDS sections, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations
• Develop, update, and monitor individualized resident care plans based on assessment findings
• Prepare and distribute the MDS schedule to the interdisciplinary team
• Transmit MDS data to the state weekly (or as required) and maintain organized, accurate records
What We’re Looking For
• Current Registered Nurse (RN) license in the state of Iowa (preferred)
• Minimum of 5 years of experience as an MDS Coordinator / RNAC
• Prior experience working as a Coordinator in a skilled nursing or long-term care setting
• Strong working knowledge of MDS processes, assessment timelines, CAAs, and care planning requirements
