Position Overview:
The Registered Nurse Care Coordinator is essential in enhancing member health outcomes through effective implementation of the Value-Based Care Management program via high-quality telephonic case and care management. This role involves collaboration with members, caregivers, providers, and interdisciplinary care teams to develop and execute impactful care plans for CareFirst members.
Key Responsibilities:
• Engage with members, caregivers, and providers over the phone to craft comprehensive care plans, pinpointing key interventions to meet members' needs.
• Act as a vital member of the care team, collaborating with primary care providers, specialists, and other healthcare professionals to achieve health goals through cooperative care plans.
• Regularly assess members' care needs and their progress, making necessary adjustments to care plans based on evolving health status and member preferences.
• Provide patient education in line with care standards, utilizing the most effective communication strategies tailored to individual members.
• Identify community resources and evaluate Social Determinants of Health to enable appropriate referrals.
• Assist members in coordinating additional tests, imaging, and specialist consultations as necessary.
• Conduct medication reconciliation to monitor effectiveness and identify potential drug interactions or side effects.
• Oversee transitions of care, ensuring seamless handoffs between healthcare providers as member needs evolve.
• Maintain thorough documentation to reflect the quality of care plans, recording members' progress and challenges in reaching their goals.
• Adhere to the guidelines of the Value-Based Care Management Program and achieve productivity and quality standards established by leadership.
• Engage in required training and actively participate in team meetings for ongoing clinical development.
• Stay updated on clinical advancements through self-directed learning.

Qualifications:
• Current multi-state compact Registered Nurse license in residence state required; BSN preferred.
• Training in motivational interviewing is highly desirable.
• Minimum of 3-5 years of clinical experience in telephonic case management or related fields.
• Competence in using electronic medical records, word processing, spreadsheets, and presentation software.
• Comfortable with various communication technologies, including Microsoft Teams, Word, Excel, and Outlook.
• The role requires a home office that meets HIPAA standards along with appropriate internet connectivity.
• Effective communication skills are essential for interaction with team members, medical professionals, and plan representatives.
• Aptitude for analyzing and interpreting scientific and technical materials to cater to diverse audiences.
