Your Responsibilities as a Registered Nurse Care Coordinator (RNCC):
• Conduct in-person patient interviews and perform thorough physical assessments.
• Oversee the delivery and ongoing evaluation of the patient's care plan, coordinating with home health aides, LPNs, RNs, and other caregivers.
• Communicate patient updates and collaborate with healthcare providers to ensure timely care adjustments.
• Educate patients and their families on essential home healthcare practices including wound care, IV management, and medication protocols.
• Work proactively to reduce readmissions by promoting preventive care and ensuring continuity of patient care.

Qualifications Required:
• Active, unrestricted license to practice as a Registered Nurse (RN).
• Current certifications in CPR, AED, BLS, and First Aid.
• Reliable transportation for visits to care locations.
• A minimum of one year of RN experience in home health or a similar field; case management experience is a plus.
• Familiarity with OASIS documentation is preferred.
• Experience with trach and/or ventilator care is beneficial, but not mandatory.
