
Registered Nurse Care Coordinator - Case Management in Wolcott, CT

As a licensed RN Care Coordinator, you'll leverage your nursing expertise to assess and treat patients across the continuum of care. Your primary responsibilities will include:
• Developing and implementing effective discharge plans in collaboration with an interdisciplinary healthcare team.
• Identifying and addressing complex clinical care needs to ensure seamless access to post-acute care.
• Working alongside healthcare providers to eliminate discharge barriers and optimize hospital stays.
• Providing clinical leadership and ensuring daily goals align with patient care plans.
Qualifications:
We are seeking candidates who possess:
• A degree from an accredited nursing school; BSN is preferred.
• Previous experience in an acute care or diverse healthcare setting.
• A Valid Registered Nurse License in the State of Connecticut (Case Management Certification is a plus).
Required Skills:
• Strong understanding of medical terminology, illness progression, and disease processes.
• Excellent communication, negotiation, and conflict resolution capabilities.
• Proficient in clinical documentation and consultation along the care continuum.
• A collaborative spirit to work with various healthcare professionals, ensuring a smooth transition of care.
• Ability to multitask effectively in a fast-paced healthcare environment.
• Experience in addressing complex psychosocial needs by utilizing community resources.
