Are you a passionate Registered Nurse (RN) looking to make a difference in the lives of individuals receiving Medicaid/Medicare services? We are seeking an experienced RN Care Coordinator to join our team. This position requires an active RN license in South Carolina or a compact license.
The RN Care Coordinator will play a key role in assessing member needs, creating care plans, and connecting individuals to vital community resources. If you thrive in a fast-paced environment and have excellent computer skills, this position could be an excellent fit for you!
Schedule: Monday through Friday, 8:00 AM to 5:00 PM EST (with flexibility to work 1 to 2 nights a week until 6 PM EST). Enjoy no weekends or holidays!
Essential Job Duties:
• Conduct comprehensive assessments and determine eligibility for care management based on clinical judgment.
• Develop and implement individualized care coordination plans in partnership with members, caregivers, and healthcare professionals.
• Perform ongoing monitoring of care plans, documenting interventions and outcomes efficiently.
• Manage a caseload for regular outreach, enhancing continuity of care.
• Facilitate interdisciplinary team meetings to ensure coordinated member care.
• Utilize motivational interviewing techniques to educate and support members in their care journey.
• Assist members in overcoming barriers to care through effective coordination and resource connection.
• May conduct medication reconciliation and manage complex cases as required.
• Expect 25-40% local travel in line with state or contractual requirements.
Required Qualifications:
• A minimum of 2 years of experience in healthcare, preferably in care management or a clinical setting.
• Active and unrestricted RN license in South Carolina.
• Valid driver’s license and reliable transportation.
• Understanding of EMR systems and HIPAA regulations.
• Proven knowledge of community resources.
• Excellent communication and problem-solving skills.
• Proficiency in Microsoft Office and navigating online databases.
Preferred Qualifications:
• Certified Case Manager (CCM) certification.
