In this role, you will coordinate long-term care needs for patients within the community, working in the field and ensuring each patient receives individualized care. As a Health and Social Services Care Coordinator, you will play a pivotal role in our Integrated Care Model by sharing important information regarding member needs, advocating for optimal care, and ensuring that patients access the right services tailored to their needs.
If you're located on Molokai, HI, you will have the flexibility to work remotely* while tackling challenging issues. The position requires approximately 75% local travel.
Primary Responsibilities:
• Evaluate, plan, and implement personalized care strategies directed at the most appropriate, least restrictive levels of care.
• Initiate referrals for social service programs, encompassing financial, psychosocial, community, and state-assisted services.
• Manage the care plan throughout the care continuum as the single point of contact.
• Communicate necessary health-related information to all stakeholders, ensuring quality coordinated care and prompt service delivery to all members.
• Advocate for patients and families, ensuring their needs and preferences are fully represented and supported by the healthcare team.
• Conduct visits to members in their homes or long-term care facilities, demonstrating flexibility and patience in varying situations.

Required Qualifications:
• Current, unrestricted RN license in the state of Hawaii.
• 2+ years of experience in a community health setting or a related healthcare role.
• Experience working directly or collaborating with services for long-term care, home health, hospice, public health, or assisted living.
• Intermediate proficiency with MS Word, Excel, and Outlook.
• Willingness or ability to travel within the assigned region to visit Medicaid members in various settings, including homes, community centers, hospitals, or provider offices.
• Access to reliable Internet via landline.
• Driver's license and reliable transportation.
Preferred Qualifications:
• Bachelor's degree or higher.
• CCM (Certified Case Manager) certification.
• Case Management experience.
• Exposure to discharge planning.
• Experience with utilization review, concurrent review, or risk management.
• Familiarity with electronic charting.
• Experience arranging community resources.
• Background in Public Health Nursing.
• Field-based work experience.
• Experience managing populations with complex medical or behavioral needs.
• Background in mental health or experience working with serious mental illness.

The hourly pay for this role will range from $28.27 to $50.48 based on full-time employment, subject to several factors including local labor markets and experience.
