Job Summary • Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care. • Collect and relay information to providers regarding hospital and clinic patients enrolled in approved programs. • Develop individualized patient education based on assessed needs and implement care plans. • Coordinate patient care in collaboration with an inter-professional team and community partners. • Manage disease registries and follow up with patients to ensure they meet clinical guidelines and goals. • Provide services that emphasize health and wellness and lead healthcare initiatives. • Demonstrate competency within the full scope of nursing expertise and adhere to relevant ethical standards. Qualifications • Bachelor's degree in Nursing is preferred; required for specific locations. • Graduate from an accredited nursing program. • Minimum of two years' clinical experience preferred. • Must hold a valid, unencumbered RN license in the state of practice. • Basic Life Support (BLS) certification required within six months of employment. • Valid driver's license is a must.
