As a Registered Nurse Care Coordinator at CenterWell Home Health, you will have the rewarding opportunity to significantly impact the lives of patients every day. By providing personalized, one-on-one care, you will help individuals regain their independence in the comfort of their own homes. You'll work closely with a dedicated team of physicians and clinicians to develop and manage tailored care plans that support patients on their recovery journey.
Key Responsibilities:
• Conduct comprehensive admission assessments, case management, and follow-up nursing visits for home health patients.
• Administer continual care and case management, ensuring follow-up as guided by the Clinical Manager.
• Collaborate with physicians to formulate effective treatment plans based on initial assessments and patient needs.
• Deliver hands-on care and provide education to patients according to physician orders and clinical guidelines.
• Revise care plans in consultation with physicians, ensuring compliance with assessments and regulatory requirements.
• Coordinate care efforts with various healthcare professionals, including therapists and home health aides.
• Continuously monitor and report on patient conditions to physicians and the Clinical Manager.
• Support patients and their families in achieving optimal health outcomes by implementing individualized care plans.
• Supervise Home Health Aides and Personal Care Workers, offering clear care instructions and ensuring accurate documentation.
• Facilitate patient discharges in consultation with physicians and prepare necessary clinical documentation.
• Maintain meticulous medical records, including patient contacts, medication changes, and progress updates.
Required Qualifications:
• Diploma, Associate, or Bachelor's Degree in Nursing.
• A minimum of one year of nursing experience preferred, with a strong background in med-surg, ICU, ER, or acute care.
• Home health experience is advantageous.
• Must hold a current and unrestricted Registered Nurse license and CPR certification.
• Exceptional organizational skills and effective communication abilities.
• Valid driver's license, auto insurance, and reliable transportation are mandatory.
Compensation:
The pay range is $45.00 - $63.00 per visit/unit, corresponding to an annual salary of $70,500 - $96,900. This estimate reflects base pay for full-time employment (40 hours per week) and may be adjusted based on location and individual qualifications.
