
Registered Nurse Care Manager in Arlington, MA
Job Description
As a Registered Nurse Care Manager, you will oversee and coordinate high-quality clinical services for patients receiving home health care. You will lead an interdisciplinary team, ensuring that care plans align with physician directives and comply with all relevant laws and regulations.
Key Responsibilities:
• Deliver clinical services within the guidelines of nursing practice as mandated by state laws, while collaborating with the healthcare team.
• Manage patient referrals, assign clinicians, evaluate patient orders, and schedule start of care visits.
• Coordinate patient home health benefits and ensure continued insurance approvals.
• Continuously assess patient needs, ensuring individualized care aligns with eligibility criteria and physician orders.
• Review patient assessments and plans of care daily, providing support and consultation to clinicians as needed.

Required Qualifications:
• Current unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and reliable transportation.

Preferred Qualifications:
• Experience in home care settings.
• Ability to work independently.
• Strong communication, writing, and organizational skills.
Your compensation will depend on various factors, including local labor market conditions, education, experience, and certifications. The salary range for this position is $58,800 to $105,000 annually for full-time roles.
