As an RN Care Coordinator, you will collaborate with teams to provide essential education and support regarding social determinants of health, managing high-risk patients through needs assessment, care plan development, and service coordination. Additionally, you will assist patients facing various mental health challenges in accessing necessary resources. You will advocate for patients and their families within the healthcare landscape, working proactively with the healthcare team to maintain optimal health outcomes. You will have the opportunity to innovate and contribute to new programs focused on chronic condition education and initiatives aimed at reducing emergency department utilization and hospital readmissions. This role involves both independent work and collaboration within the Care Coordination team at The Corvallis Clinic Asbury Building.

This position follows a hybrid schedule with three in-office days per week.

Principal Responsibilities:
• Embrace and promote a culture aligned with The Corvallis Clinic’s Service and Behavioral Standards.
• Collaborate effectively with the interdisciplinary team as part of our Patient Centered Primary Care Medical Home.
• Utilize the nursing process for assessments, planning, interventions, implementations, and evaluations.
• Develop and implement new programs to enhance patient care as needed.
• Support clinical changes in accordance with Primary Care First and other Quality contracts.
• Daily review of inpatient utilization and discharge planning, addressing issues with the healthcare team.
• Facilitate early interventions to prevent hospitalizations of high-risk patients and coordinate community support services.
• Arrange follow-up mental health care and resource navigation.
• Coordinate follow-up care with Home Health Services and Skilled Nursing Facility services.
• Manage care coordination for patients entering hospice or palliative care.
• Contact patients shortly after hospital discharge to ensure care needs are met and verify readiness for home.
• Review emergency department usage and engage with patients to educate them about alternative care options.
• Contribute to initiatives aimed at reducing emergency department utilization.
• Make appropriate referrals to health plans and ensure service coordination accuracy.
• Collaborate with individual providers to facilitate changes in practice when necessary.
• Engage with primary care teams to discuss high-risk patient management and quality data.
• Pursue continuous improvement in care quality through ongoing education and self-evaluation.
• Assist in training and orienting new employees.
• Work collaboratively with patients in both individual and group settings.
• Attend patient appointments and participate in primary care huddles to support care coordination.
• Complete individualized patient Care Plans for the medical home.

Required Qualifications:
• Graduation from an accredited nursing program and a minimum of one (1) year of nursing experience.
• Active and unencumbered State of Oregon license as a Registered Nurse.
• Current Basic Life Support (BLS) certification or ability to obtain certification within 90 days of hire.

Knowledge and Skills:
• Case Management certified or willingness to achieve certification within two years.
• Proficient understanding of the nursing process.
• Adaptability, flexibility, and strong time management skills.
• Excellent customer service skills and ability to work collaboratively in a multidisciplinary team.
• Ability to manage multiple tasks efficiently in a fast-paced environment without compromising quality.
• Understanding of chronic health conditions and their extensive impact.
• Self-starter with a strong sense of accountability.
• Effective communication skills, both written and verbal.
• Adept at identifying complex problems and formulating reasonable solutions.

We offer a competitive salary which will range from $58,800 to $105,000 annually for full-time employment, along with a comprehensive benefits package including stock purchase options and 401k contributions (subject to eligibility).
