Registered Nurse Care Manager for Behavioral Health in Bowling Green, KY

Job Summary As a Registered Nurse Care Manager specializing in Behavioral Health, you will play a crucial role in enhancing member care through effective care management and coordination. You will collaborate with a multidisciplinary team to ensure seamless delivery of care that contributes to quality outcomes and cost-effective care solutions. Essential Job Duties: • Conduct comprehensive assessments of members promptly to identify who may benefit from care management based on clinical insights, health changes, and psychosocial factors. • Develop and implement tailored care coordination plans in partnership with members, caregivers, and healthcare professionals to meet individual needs and aspirations. • Engage in telephonic, face-to-face, or home visits as necessary to provide personalized support. • Monitor and evaluate the effectiveness of care plans, documenting outcomes and making necessary adjustments to ensure goal achievement. • Manage an ongoing caseload of members, providing regular outreach and management to foster continuous support. • Facilitate the integration of services across behavioral health, long-term services, and community resources to enhance care continuity. • Lead interdisciplinary care team meetings and foster informal collaborations for improved outcomes. • Utilize motivational interviewing techniques and Molina clinical guidelines to educate and encourage lifestyle changes during member interactions. • Identify barriers to care and provide assistance to members in overcoming challenges to access services. • Offer consultations, resources, and insights to colleagues as needed. • Handle complex cases involving intricate medication regimens, including medication reconciliation. • Be prepared for 25-40% travel as required by contractual obligations. Required Qualifications: • A minimum of two years of experience in healthcare, ideally in care management or a behavioral health setting. • A current and unrestricted Registered Nurse (RN) license in your state of practice. • A valid driver’s license, reliable transportation, and appropriate auto insurance for job-related travel. • Proficient understanding of electronic medical records (EMR) and HIPAA regulations. • Knowledge of community resources to support member care. • Detail-oriented with a proactive work approach. • Ability to adapt communication styles to diverse populations and various situations. • Strong independence and self-motivation skills. • Excellent communication skills, both verbal and written, to maintain professional relationships. • Exceptional time-management and prioritization abilities, adept at handling multiple projects and adapting to change. • Strong problem-solving and critical-thinking capabilities. • Proficient in the Microsoft Office Suite and capable of navigating online portals and databases. Preferred Qualifications: • Certified Case Manager (CCM) certification.
