<li>Develop individualized, person-centered Care Plans in collaboration with clients and families</li>

<li>Perform supervisory and competency visits</li>
<li>Support caregivers with clinical guidance and best practices</li>

<li>Monitor vitals, condition changes, and client status</li>
<li>Document accurately in our HIPAA-compliant EMR</li>
<li>Escalate concerns and changes in condition promptly</li>

<li>Partner closely with Client Care Managers, Care Coordinators, and Scheduling</li>
<li>Ensure seamless, client-centered care transitions</li>

<li>Maintain audit-ready care plans and supervisory documentation</li>
<li>Ensure compliance with state home-care regulations and agency standards</li>

<strong>Active Registered Nurse (RN) license</strong> in <strong>Maryland and/or Washington, DC</strong>
</li>
<li>
<strong>BSN or related healthcare degree</strong> — <em>or</em> <strong>2+ years of home care / geriatric nursing experience</strong>
</li>
<li>Strong clinical assessment and critical-thinking skills</li>
<li>CPR/First Aid certification</li>
<li>Valid driver’s license and reliable transportation</li>
<li>Professional, compassionate communication with clients, families, and field staff</li>
