What You Will Do: • Serve as a Change Agent: Utilize your nursing expertise to connect mobile healthcare resources and enhance patient outcomes. • Conduct Telephonic Patient Assessments: Assess patient needs remotely with evidence-based protocols to help them navigate the healthcare system. • Empower & Educate: Provide essential health education and assist patients in understanding their care options across the healthcare continuum. • Utilize Technology: Engage in telemonitoring and track patient progress using innovative tools. • Collaborate and Lead: Contribute to team care plans and mentor colleagues to advance new approaches in healthcare delivery. • Advocate for Our Mission: Represent our innovative healthcare programs in the community, reaching those who require support the most. • Facilitate Training: Educate others on delivering better integrated healthcare. • Deliver Exceptional Service: Manage your resources and documentation efficiently while providing outstanding customer support. Minimum Requirements: • Valid RN Compact State License. • 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment. • Experience with clinical data, ethical principles, and patient advocacy. • Graduate of an accredited nursing program. • Current CPR certification. • A Bachelor’s or Master’s degree in Public Health, Healthcare Administration, or related fields focused on community health is a plus.
