As a Registered Nurse Case Coordinator, you will play a key role in performing in-home admissions, conducting supervisory visits, fulfilling office duties, and executing semi-annual or annual on-site assessments of field caregivers. Our team is focused on upholding compliance with client needs assessments and service plan standards in accordance with state regulations. This position typically has office hours from Monday to Friday, 9:00 AM to 5:00 PM, though flexibility is required to meet business needs. A Typical Day for a Registered Nurse Case Coordinator Includes: • Conducting initial evaluation visits and regularly reassessing clients' nursing needs. • Counseling clients and their families on nursing and related care needs. • Coordinating and developing service plans based on client assessments and admissions. • Supporting and supervising field staff, ensuring effective implementation of new Medicaid Waiver programs. • Ensuring the delivery of quality care by creating, implementing, monitoring, and enforcing quality assurance policies and procedures. • Assisting branch teams with pre-service training and orientation, along with in-service training for caregivers. • Providing additional support as necessary, per assignments from the Branch Manager. To be considered for this role, you should possess: • A valid RN license in the applicable state. • A valid driver's license. • Access to reliable and insured transportation. • Willingness to travel within surrounding counties 2-3 days per week during regular hours. • Availability to be On-Call for caseload-related emergencies. • Preferred nursing experience. Registered Nurse Case Coordinators must comply with state background screening requirements.
