To view more details about the comprehensive benefits package we offer to eligible employees visit our website - Job Description The Case Manager (RN) reviews all direct and Emergency Department admissions or observation patients (preferably at the time of admission) for correct status.  In addition, they assess patient clinical data for continued stays, discharges and plans of care in all patient care units.  This individual will promptly provide clinical reviews required and/or requested by insurance payers.   Case Managers record and track data to assist in providing the best quality of care to our patients and to assure that resources are utilized appropriately.  They assist in ensuring that Medicaid, Emergency Department Information Exchange, and Patients Requiring Coordination (PRC) patients who are seen in the Emergency Department receive timely follow-up with Primary Care Providers.
