As a Registered Nurse Case Management Specialist, you will elevate the practice of nursing and enhance patient outcomes with your expertise. Role Overview: As a critical member of our healthcare team, you will coordinate medical services and discharge planning for specific patient populations. Your duties will include: • Providing orientation to patients, families, and caregivers regarding the case management role and relevant policies. • Acting as the main coordinator for patient/family/caregiver education to foster active participation in their care. • Ensuring that the treatment plan aligns with the strengths and preferences of the patient. • Documenting comprehensive assessments, communicating patient needs and preferences clearly. • Promoting consistent, positive interactions that focus on unparalleled patient service. Qualifications: Required Licensure/Certifications: • Registered Nurse (RN) licensure in Texas or as a Compact State. Required Education/Experience: • Associate's Degree or Bachelor of Science in Nursing (BSN). Preferred Qualifications: Three to five years of clinical experience in an acute healthcare setting, with a recent role in acute care case management. Strong knowledge of case management philosophies, utilization review, discharge planning, and insurance processes is highly desirable. Exceptional critical thinking skills and the ability to communicate effectively across different socioeconomic backgrounds are essential.
