As a Registered Nurse Case Management Specialist, you will elevate the practice of nursing and enhance patient outcomes with your expertise. Role Overview: As a critical member of our healthcare team, you will coordinate medical services and discharge planning for specific patient populations. Your duties will include: • Providing orientation to patients, families, and caregivers regarding the case management role and relevant policies. • Acting as the main coordinator for patient/family/caregiver education to foster active participation in their care. • Ensuring that the treatment plan aligns with the strengths and preferences of the patient. • Documenting comprehensive assessments, communicating patient needs and preferences clearly. • Promoting consistent, positive interactions that focus on unparalleled patient service. Qualifications: Required Licensure/Certifications: • Registered Nurse (RN) licensure in Texas or as a Compact State. Required Education/Experience: • Associate's Degree or Bachelor of Science in Nursing (BSN). Preferred Qualifications: Three to five years of clinical experience in an acute healthcare setting, with a recent role in acute care case management. Strong knowledge of case management philosophies, utilization review, discharge planning, and insurance processes is highly desirable.
