Ref #: M-RN-293430 Join our team in Juneau, AK as a full-time Registered Nurse focused on case management and specialty services. This day-shift role starts ASAP and requires an active RN license in the practicing state. You’ll assess patient needs and create individualized care plans for specialty populations, coordinate care among specialists, primary care providers, and multidisciplinary teams, and monitor progress—adjusting plans as necessary. Patient and family education on treatments, resources, and disease management is essential, as is maintaining accurate documentation and ensuring compliance with regulatory and quality standards. Qualifications: active RN license; experience in case management, specialty nursing, or care coordination preferred; strong clinical judgment, organizational ability, and communication skills; BLS required. CCM or other specialty case management certifications are a plus.
