Description:
The Registered Nurse Case Manager is essential in delivering skilled nursing care to terminally ill patients through continuous assessment and tailored care. This role involves planning, implementing, and evaluating nursing interventions to ensure that patients receive the best possible support.
Key Responsibilities:
• Utilize clinical tools and data to conduct thorough assessments of patient needs.
• Develop individualized and comprehensive case management and treatment plans.
• Educate patients and caregivers about available resources, benefits, and services.
• Document complex medical information to enhance patient outcomes.
• Adhere to case and quality management processes in compliance with regulatory guidelines and company protocols.
• Communicate effectively with patients, healthcare providers, and other stakeholders through strong written and verbal communication skills.
• Respond promptly to the needs of patients and families, demonstrating empathy and professionalism.
• Possess a general understanding of case management practices, including knowledge of relevant products and services.
• Exhibit basic computer proficiency, particularly with the Microsoft Office suite.
• Build and maintain professional relationships with patients, families, and healthcare providers.
• Provide care and support during on-call hours as scheduled.
Schedule: Monday - Friday, 8:00 am - 4:30 pm, with weekend rotation as required.
Requirements:
• Graduate of an accredited School of Nursing with a current multi-state RN license.
• Minimum of one year of experience in a patient care role within a hospital, nursing home, or similar healthcare setting preferred.
• Exhibit qualities of empathy, responsibility, and dependability.
• Demonstrate effective communication skills with team members, patients, providers, and family members.
• Ability to work independently with minimal supervision.
• Showcase leadership and teaching capabilities.
• Must possess a valid driver's license, auto insurance, and have the ability to visit patient homes and facilities.
