Registered Nurse Case Manager in Charleston, WV

**Overview:** We are seeking a highly skilled and experienced Registered Nurse Case Manager to join our team. As a Registered Nurse Case Manager, you will collaborate with the healthcare team to enhance patient care services, coordinate patient needs, and ensure high-quality patient care. **Responsibilities:** • Intervene with physicians and ancillary departments concerning clinical and utilization issues to ensure optimal patient outcomes. * Identify tests, procedures, and interventions earlier to advance the plan of care. * Provide real-time interventions to prevent delays and ensure compliance and revenue integrity with healthcare regulations. * Evaluate complex patients for needs and barriers to discharge and potential re-admissions. * Assist with transfers to other facilities. * Provide code 44 documents to Medicare patients when applicable. * Act as a resource to assist others with patient care management. * Ensure patient rights are sustained and safeguard patient privacy and confidentiality. * Assist social workers with medication prior authorizations when necessary. * Monitor trends and report practice issues of the multidisciplinary healthcare team that impacts finances and quality care issues.
