Registered Nurse Case Manager
Job Summary of Registered Nurse Case Manager:
You will organize staffing and oversee clinical care to meet resident needs with compassion and quality. Flexible 8 or 12 hour shifts are available, with full time, part time, and per diem options. Excellent benefits, including Aetna Health coverage starting the first of the month after hire, are offered for those working at least 22.5 hours weekly. A sign on bonus up to $5,000 is available.
Job Duties and Responsibilities of Registered Nurse Case Manager:
• Organize, oversee, and supervise unit activities and staff assignments, demonstrating clinical expertise aligned with nursing practice standards.
• Respond promptly and appropriately to patient needs.
• Monitor, document, and evaluate vital signs, treatments, and patient responses.
• Administer medications and treatments as required.
• Communicate plans, concerns, and updates to team members, residents, families, and medical staff.
• Implement disciplinary actions per facility policy.
• Collaborate with interdisciplinary teams, physicians, and external vendors.
• Conduct RN assessments following incidents or changes in resident condition.
• Lead emergency responses including fire alarms and elopement situations.
• Maintain accurate nursing reports, staffing sheets, and census records.
Qualifications and Experience of Registered Nurse Case Manager:
• Current RN license in good standing in New York State.
• Experience with geriatric, short-term rehab, or Alzheimer’s patients preferred.
• Progressive nursing experience with supervisory background preferred.
• Committed to providing exceptional, compassionate care.
• Excellent communication, interpersonal, customer service, and organizational skills.
• Ability to work effectively under pressure and collaborate with all staff.
• Patience and willingness to assist residents and co-workers.
• COVID 19 vaccinations encouraged but not mandated; available per CDC guidance.
