General Summary of Position

Join our dedicated Case Management Team as a Registered Nurse Case Manager specializing in discharge planning. In this impactful role, you will utilize your clinical expertise to enhance care coordination and optimize patient discharges, providing quality, cost-effective, and patient-centered care throughout their hospital journey. Your commitment to individualized patient outcomes will play a crucial role in improving care delivery and resource utilization.

Primary Duties and Responsibilities
• Contribute to achieving department goals while adhering to established policies, quality standards, and safety regulations.
• Collaborate with a multidisciplinary healthcare team to develop and coordinate comprehensive plans of care.
• Engage with patients, families, and external stakeholders to address care issues and facilitate goal achievement.
• Develop and implement holistic care plans that encompass long- and short-term goals, involving family and caregivers in the process.
• Regularly evaluate and document patient responses and outcomes, recommending care plan adjustments when necessary.
• Stay informed on regulations, admission criteria for different care settings, and reimbursement methods for various levels of care.
• Manage a diverse caseload of patients, identifying and mobilizing necessary resources for effective care delivery.
• Commit to your professional growth in managed care and clinical practice improvements.
• Drive patient care through collaborative plan execution, identifying and overcoming barriers to optimal outcomes.
• Participate in performance improvement initiatives and assist in program evaluations through data analysis.
• Conduct comprehensive assessments with interdisciplinary teams to identify and address medical, psychosocial, and financial needs.

Minimal Qualifications

Education
• Associate's degree in Nursing (ADN) required.
• Bachelor's degree in Nursing (BSN) preferred.

Experience
• 2-3 years of clinical experience required.
• 1-2 years of case management experience preferred.

Licenses and Certifications
• RN - Registered Nurse - State Licensure or Compact State Licensure required.
• CCM - Certified Case Manager preferred.

Knowledge, Skills, and Abilities
• Proficient in computer use for data entry and retrieval.
• Familiarity with Microsoft Word, Excel, and PowerPoint.
• Strong verbal and written communication skills.
• Adept at analyzing departmental productivity reports.
• Exceptional interpersonal skills required.

This position offers a competitive hiring range of USD $89,065.00 - USD $162,801.00 per year.
