Role Description This is a full-time, on-site position for a Registered Nurse Case Manager, based in Gladstone, OR. Responsibilities include coordinating and managing patient care, creating individualized care plans, performing regular patient assessments, and collaborating with interdisciplinary healthcare teams to meet patient needs. The Registered Nurse Case Manager will also provide patient education, oversee transitions between care settings, and ensure compliance with regulatory standards and best practices. Qualifications • Strong clinical skills in nursing assessment, care planning, and patient education • Proficiency in case management, care coordination, and interdisciplinary team collaboration • Knowledge of home healthcare processes, practices, and regulatory compliance • Excellent organizational, time management, and communication skills • Current Registered Nurse (RN) license in the state of Oregon • Experience in home health or community-based care is preferred • CPR certification and a valid driver’s license with reliable transportation
