Schedule: Monday-Friday 8a-5p with only 2 holidays on call per year

Your Responsibilities:
• Oversee a patient and family caseload, handling all aspects of assessment, eligibility determination, admission processing, planning, implementation, and evaluation within the nursing process.
• Gather essential data regarding the patient's physical, psychological, social, and spiritual needs and coordinate with the interdisciplinary team for effective care.
• Communicate effectively with physicians and team members to ensure coordinated and optimal patient care.
• Conduct regular visits for ongoing assessment of patient needs and updates to care plans.
• Perform clinical pronouncement of death when necessary and provide compassionate support to grieving families.
• Maintain precise and timely documentation of patient records and care plans.
• Supervise Home Health Aides to ensure compliance with care standards.
Qualifications:
• Graduation from an accredited registered nursing program.
• Active registered nursing license in the relevant state.
• Current CPR and First Aid certifications.
• Familiarity with quality assessment and assurance protocols.
• Valid driver's license and proof of insurance required.
