Job Summary and Qualifications In the role of Case Manager, you will be the crucial link guiding patients and families throughout their rehabilitation journey. Your responsibilities will include coordinating care, connecting resources, and creating care plans tailored to each patient's needs by collaborating with healthcare professionals to ensure seamless communication and quality care. Your responsibilities will include: • Educating patients and families about program orientation, rehabilitation philosophies, insurance benefits, patient rights, and discharge criteria. • Facilitating educational sessions to engage patients and families in discussions about treatment planning and recovery goals. • Developing and implementing individualized treatment strategies that align with patients’ strengths, needs, and recovery objectives. • Conducting psychosocial assessments and discharge planning evaluations while ensuring clear communication among care team members. • Working closely with the Rehab Program Director and the interdisciplinary team for cohesive and supportive patient care. • Embodying HCA Healthcare’s values of compassion, respect, and excellence in every patient interaction. What qualifications you will need: • A valid Registered Nurse (RN) license obtained within 30 days of starting employment. • A Bachelor’s Degree or equivalent work experience.
