Description Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking a Registered Nurse Case Manager in Clewiston, Florida. This is a full time outpatient Registered Nurse Case Manager opportunity focused on coordinating and managing patient care across complex outpatient and discharge planning needs. The role supports patients, families, and interdisciplinary teams to ensure quality outcomes, appropriate utilization of services, and continuity of care. Core duties include developing and evaluating individualized patient care plans, coordinating discharge planning, performing prospective, concurrent, and retrospective clinical reviews, and serving as a patient and family advocate throughout care transitions. The role may also include in home or hospital evaluations as needed.
