Department Description
Join our dedicated Inpatient Case Management team, where we provide vital psychosocial assessments and referrals for adults and families facing psychiatric illness, substance abuse, and medical issues. In this role, you'll be conducting casework and therapy within the hospital setting, making a real difference in patients' lives.
As an RN Case Manager, you will work under the supervision of the Director of Care Management, offering clinically-based case management that ensures effective and efficient patient care in alignment with the Centers for Medicaid and Medicare Conditions of Participation. Collaborate with a dynamic healthcare team to determine the best use of resources, confirm medical necessity for admissions and continued stays, and develop comprehensive post-acute care plans tailored to patients' unique needs.
Qualifications/Requirements:
• BSN preferred
• Current MA RN Licensure
• Current American Heart Association (AHA) for Healthcare Providers BLS (Basic Life Support) certification required
• Certification in Case Management preferred
