Pride Health is currently seeking candidates for RN Case Manager that will provide support for our client's healthcare systems in Detroit, Michigan. This is a contract assignmentwith a high likelihood of extension and a great way to start working with a top-tier healthcare organization! Job Description: Job Title: RN Case Manager Location: Detroit, Michigan Schedule: 40 hours/Week Shift: Days (8:00 AM – 4:30 PM) Contract Length: 13 Weeks Pay Range for Traveler: $2,250.00- $2,328.00 Weekly DOE Job Summary • Provides collaborative assessment, planning, care coordination, facilitation, evaluation, and advocacy • Supports patients/families with complex medical needs across the care continuum • Promotes patient safety, quality care, and cost-effective outcomes Key Responsibilities • Perform comprehensive assessments of patient and caregiver needs (biomedical, psychological, social, functional) • Develop personalized patient-centered care plans • Provide advocacy, education, communication, and resource coordination • Use motivational interviewing and self-management techniques to overcome barriers • Provide counseling for treatment decisions and end-of-life planning (Advanced Care Planning) • Coordinate safe transitions between care settings • Collaborate with interdisciplinary teams and facilitate care conferences • Monitor patient progress and adjust care plans as needed • Participate in process improvement and risk reduction activities • Handle cases related to abuse/neglect, domestic violence, elder abuse, and sexual assault • Support department goals and complete other duties as assigned REQUIRED Experience • Minimum 5 years Acute Inpatient Case Management experience • Does NOT include telephonic case management, utilization management, or therapy experience • Acute discharge planning experience • Recent experience (within last 2 years) in acute inpatient case management • Epic EMR experience Required Certifications & Licensure • BLS certification (AHA required) • Valid Michigan license: • RN (unrestricted MI RN license) OR • LMSW (unrestricted MI licensure) About Pride Health: Pride Health is Pride Global healthcare staffing branch, providing recruitment solutions for healthcare professionals and the industry at large since 2010.
