Job Description
Join our dedicated team as a Registered Nurse Case Manager, where you will provide compassionate nursing care to terminally ill patients based on ongoing assessments of their needs. You will have the opportunity to create impactful individualized care plans, ensuring optimal health outcomes for patients and their families.
Key Responsibilities:
• Utilize clinical assessment tools and data to evaluate patient needs comprehensively.
• Develop, implement, and adjust tailored case management and treatment plans.
• Educate patients and caregivers about available services, benefits, and resources.
• Document intricate medical information effectively to enhance care results.
• Adhere to quality management and case processes in line with regulatory guidelines and company policies.
• Communicate effectively with patients, families, physicians, and stakeholders.
• Respond promptly to the needs of patients and families, showcasing empathy and dependability.
• Maintain a good grasp of case management practices and relevant healthcare products/services.
• Establish and nurture collaborative relationships with patients, families, and healthcare providers.
• Provide care and support during on-call hours as scheduled.
This full-time position offers a schedule of Monday to Friday, from 8:00 am to 4:30 pm, with occasional weekend rotation as needed.
Qualifications:
• Graduate of an accredited School of Nursing with a current multi-state RN license.
• Minimum one year of experience in a patient care role within a healthcare setting preferred.
• Exhibit personal qualities of empathy, responsibility, and dependability.
• Demonstrated ability to communicate effectively with team members, patients, providers, and families.
• Proven ability to work independently and take initiative.
• Strong leadership and teaching skills.
• Possess a valid driver's license and auto insurance for patient home and facility access.
