Key Responsibilities for Case Manager RN: • Coordinate and oversee patient care plans to ensure continuity and quality of care. • Perform patient assessments, including psychosocial, medical, and functional evaluations. • Collaborate with interdisciplinary teams to develop individualized care plans and monitor patient progress. • Act as a liaison between patients, families, and healthcare providers, facilitating effective communication and resource coordination. • Advocate for patient needs while ensuring compliance with healthcare regulations and standards. • Maintain accurate documentation of patient care activities in accordance with facility policies and regulatory requirements. • Educate patients and families on care plans, available resources, and post-care expectations. Case Manager RN Qualifications: • Active and unrestricted New Mexico Registered Nurse (RN) license. • Graduation from an accredited nursing program required. • BLS certification required. • At least 2 years of clinical nursing experience preferred; prior case management experience is a plus. • Strong interpersonal and communication skills, with a patient-centered approach. • Ability to work collaboratively in a team environment and independently manage a diverse caseload.
