About This Role: As a pivotal member of our Case Management Team, you will utilize your RN clinical expertise to coordinate patient care and ensure a smooth discharge process. Your mission is to facilitate high-quality, cost-effective, and patient-centered care from pre-admission to post-discharge. You will be instrumental in meeting individualized patient outcomes and monitoring care delivery during hospital stays to promote optimal clinical results. Key Responsibilities: • Achieve department goals while adhering to policies, quality standards, and safety regulations. • Work collaboratively with a multidisciplinary healthcare team to create and coordinate comprehensive care plans. • Engage with patients, families, and healthcare partners to discuss care needs and set patient goals. • Develop individualized care plans focusing on short- and long-term patient needs, incorporating input from families and caregivers. • Evaluate and document patient responses to care and recommend necessary modifications to the plan. • Stay informed about regulatory requirements and reimbursement methods for various levels of care. • Manage a diverse caseload, identifying essential resources for care implementation. • Continue your professional growth in managed care and clinical practices. • Direct patient care management while identifying barriers to optimal outcomes. • Participate in performance improvement initiatives, analyzing data for program evaluations. • Conduct comprehensive assessments in collaboration with interdisciplinary teams to understand patient needs. Minimum Qualifications: Education: • Required: Associate's Degree in Nursing (ADN) • Preferred: Bachelor’s Degree in Nursing (BSN) Experience: • Required: 2-3 years of clinical experience • Preferred: 1-2 years of case management experience Licenses and Certifications: • Required: RN - Registered Nurse licensure in the relevant state • Preferred: CCM - Certified Case Manager This position offers a competitive salary range of USD $89,065.00 - USD $162,801.00 /Yr.
