Registered Nurse Case Manager - RN in Richmond, VA

This is a full time role that will serve patients and family members in the inner south, central, and northern area of Richmond, VA. This position offers: • Competitive salary • Robust Benefit Package • Mileage reimbursement • Flexible work schedule Job responsibilities can include but are not limited to: • Administer skilled nursing for all ages in their place of residence. • Conduct patient admissions. • Conduct routine patient visits. • Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met. • Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care. • Demonstrates accountability, flexibility, respect, and integrity. • Submits all required paperwork and documentation within 48 hours. Qualifications: • Current Registered Nurse license in the state of Virginia. . • One (1) years experience in a hospice setting required as a RN Case Manager or similar role. • Must have a current Texas Driver's license and dependable transportation.
