In the Case Coordination Department, our primary responsibility is the assessment of patient needs to ensure the most appropriate level of care across the continuum. As a Registered Nurse Case Manager, you will join a team of skilled nurses focused on discharge planning and case management, providing patient advocacy throughout their care journey.

Position Summary:

As a Case Manager, your role will focus on facilitating patient throughput. You will:
• Assess patients for appropriateness of care and coordinate services across the continuum.
• Identify risk factors and engage support services.
• Perform utilization management functions and develop transition planning activities.
• Document care aspects in case management-specific applications.
• Collaborate with healthcare team members on process and quality improvement initiatives.

You will become an integral part of a collaborative nursing team with a first priority on patient care.

Qualifications:
• Current CT RN license required.
• ASN required, BSN preferred.
• Minimum of three years of acute-care and diversified clinical nursing experience, including medical/surgical nursing. Case management, home care, and managed care experience strongly considered.
• Excellent communication, negotiation, and conflict resolution skills are essential.
• Preferred knowledge of computer applications and reimbursement systems.
• Certification in case management is desired.
