As an RN Case Manager, you will conduct in-home nursing visits for clients with chronic and terminal illnesses. These visits, lasting 1-2 hours each week, include physical assessments, ensuring home safety, managing medication, and overseeing caregivers providing support in clients' homes. This role offers the opportunity to build meaningful, one-on-one relationships with patients, enhancing their health and quality of life. Your flexible schedule allows time for personal commitments, school, travel, or family activities. Qualifications: • Graduate of an accredited nursing program • BSN preferred • Valid and unrestricted RN license • At least 2 years of nursing experience, including 1 year in home care or a related field • Preferred: 1 year of supervisory or case management experience • Current CPR certification Essential Responsibilities: • Manage assigned clients' care and overall case management • Collaborate with the regional director to improve care and address any issues • Plan and evaluate care provided to clients • Lead client care conferences as necessary • Act as a local clinical resource to ensure needs are met • Work with staffing coordinators on appropriate staffing and scheduling • Ensure adherence to nursing delegation guidelines and agency policies • Participate in performance improvement initiatives and clinical documentation review • Assist with interviewing and training clinical personnel as needed • Engage in various tasks to support operations as requested Benefits Include: • Medical • Dental • Vision • 401(k) • Company Paid Short Term Disability • Flexible Spending Account (FSA) • Health Savings Account (HSA) • Paid Time Off • Voluntary Benefits Standard Rate: $40.00 Hourly plus applicable shift differentials.
