Key Responsibilities:
• Oversee a patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation as part of the nursing process.
• Gather and assess essential information regarding the patient's physical, psychological, social, and spiritual factors to understand their needs and involve the interdisciplinary team as needed.
• Communicate effectively with attending physicians, interdisciplinary teams, and outside agencies to coordinate optimal patient care.
• Conduct regular visits to reassess patient needs and update care plans accordingly.
• Perform clinical pronouncements of death and offer support to families during these difficult times.
• Maintain accurate patient records, including charts, progress notes, and timely care plans.
• Supervise and document the delivery of care provided by Home Health Aides, in compliance with Federal, State, and Company guidelines.
Qualifications:
• Graduate from an accredited registered nursing program
• Current registered nurse license in the state of practice
• Valid certification in CPR and First Aid
• Familiarity with quality assessment and assurance procedures
• Valid driver's license and proof of insurance required
