Job Description
Requirements:
• Eighteen months of recent experience in an Acute Care Case Manager setting
• Active RN License
• BLS Certifications
• Degree from an accredited nursing program

Responsibilities:
• The Case Manager RN will coordinate continuity of care for patients, acting as a liaison between the patients and their families, and the healthcare organization.
• Work in diverse settings, including HMOs, community health organizations, long-term care facilities, behavioral health programs, rehabilitation centers, schools, and case management companies.
• Promote self-managed care and utilize healthcare resources efficiently and cost-effectively.
• Ensure timely and appropriate treatment is provided to maximize health and well-being.
