Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching. • Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology. Sign-on Bonus up to $10,000.00. Days: Mon-Fri Hours: Can work 4/10's or 5/8's. Possibility to turn into after orientation period.
