About the Position
POSITION: Registered Nurse - Rheumatology/INR
EMPLOYMENT TYPE: .6 FTE (48 hours a pay period)

Job Summary:
The Clinic Rheumatology and INR RN works in the outpatient clinic setting and plays a critical role in managing the care of patients with chronic rheumatic diseases, autoimmune conditions, additionally patients receiving services for anti-coagulation; these roles are typically played out during separate shifts. The RN helps patients manage symptoms, prevent disease progression, and improve their quality of life through skilled clinical care, patient education, and support while being a general advocate for the patient. The position involves thorough care coordination, knowledge and integration of standards, resource utilization, data collection and critical thinking to ensure that patients receive appropriate treatments and follow-up care. The clinic RN role provides triage over the phone, provider in basket management and MyChart message management for patients.
Here's what you'll do when you join us
• Follows the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to function in a full scope RN role to support providers in the clinic, rheumatology, and INR patients.
• Works collaboratively with cardiologist, physicians/providers and pharmacy to manage patients on anti-coagulant therapy in accordance with organizational policies and protocols.
• Works collaboratively with clinic personnel/departments and hospital personnel/departments to support smooth patient transitions.
• Acts as a resource and team lead to LPN and CMA staff in assigned unit in accordance with the Minnesota Nurse Practice Act (NPA).
• Provides patient education following clinic visits.
• Manage Rheumatology providers' in-baskets, including phone calls, MyChart messages, refills, and relays some test results per Rheumatology provider.
• Completes patient chart preparation, including: medication, allergy, problem list, and immunization reconciliation; pending standing lab orders and medication refills; reference last office visit note and anything new since that visit (medication changes, tests, ED visits, hospitalizations, etc.); record last labs, dates of any other tests, and any referral visits; make note of ROI forms that need signing; and work with patient to obtain records needed prior to consult.
• Works collaboratively with physicians/providers and nursing personnel to triage patients over the phone, directing them appropriately with best clinical judgement.
• Works collaboratively with clinic personnel/departments and hospital personnel/departments to support smooth patient transitions.
• Provides evidence-based patient and staff education relating to clinical care.
• Completes Transitional Care Management (TCM) calls for hospital and emergency department discharged patients, when directed, ensuring patients are contacted to reduce risk of readmission and obtain TCM payment.
• Provides direction to LPN/CMA or Clinic Nursing Leader on supplies and patient education materials that need stocking.
• Follows-up with patients after visits and assists to coordinate follow-up appointments with primary or specialty care.
• Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resources people, if needed.
• Manages scheduled medication contracts for unit patients.
• Other duties in support of unit patient's plan of care and consistent with full scope RN practice in accordance with Minnesota Nurse Practice Act.
If you have these qualifications, we'd love to chat:
• Registered Nurse with an active RN license in the state of Minnesota.
• 1 year experience in chronic disease management in an outpatient setting is required.
• Knowledge of autoimmune diseases, musculoskeletal conditions, and pharmacology is preferred.
• Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing.
• Demonstrates clinical competence in providing direct patient care and patient and family education as well as assisting the physician in the delivery of patient care.
• Has current BLS certification (or is willing to obtain within two months of employment.)
• Demonstrated ability to manage the complex needs of a family medicine practice.
• Demonstrated competence in managing patients using Patient Care Protocols.
• Ability to manage change in a positive patient centered manner.
• Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, and mission.
• Maintains reliable and consistent attendance.
