As a Registered Nurse Clinical Care Manager, you will play a vital role in our Integrated Care Model. You will advocate for our members, ensuring they receive the highest quality of care tailored to their individual needs, while also overseeing the provision of services necessary to meet those needs.
Travel throughout the specified boroughs is part of this role. If you're located in New York state, you'll have the flexibility to work remotely while taking on significant challenges.
Primary Responsibilities:
• Assess, plan, and implement personalized care strategies aimed at ensuring the least restrictive and most appropriate level of care for each patient.
• Conduct the NYS UAS Assessment in members’ homes at least twice per year and as required.
• Identify and refer members to social service programs, including financial, psychosocial, and community support services.
• Serve as the single point of contact to manage the care plan throughout the continuum of care.
• Communicate essential health-related information to all stakeholders to ensure prompt, coordinated care and services for members.
• Advocate for patients and their families to ensure their needs and preferences are accurately represented and supported by the healthcare team.

Required Qualifications:
• Current, unrestricted RN license for New York State.
• At least 2 years of relevant clinical work experience.
• Minimum of 1 year of experience in community case management with a focus on care coordination for individuals with complex needs.
• Experience in long-term care, home health, hospice, public health, or assisted living settings.
• Proficient in MS Word, Excel, and Outlook.
• Possession of a New York state issued ID or ability to obtain one before hire.
• Residence in New York State.
• Willingness to travel within the assigned region to visit Medicaid members in their homes or other locations, including community centers, hospitals, or providers' offices.
Preferred Qualifications:
• Degree in behavioral health or clinical field.
• Experience with electronic charting systems.
• Knowledge of arranging community resources.
• Fieldwork experience.
• Background in managing populations with complex medical or behavioral needs.
• Proficient in utilizing UASNY.
