Registered Nurse in Edgewater, NJ

Schedule: Full-time | Part-time

Position Summary

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to provide and coordinate cost-effective, high-quality care for patients in a community-based setting. The RN will work closely with physicians and interdisciplinary teams to ensure care plans are tailored, compliant, and responsive to patient needs.

Key Responsibilities
• Provide and coordinate patient care that is cost-efficient, physician-approved, and reimbursable under the patient’s insurance plan
• Admit patients into the program in a timely and accurate manner
• Use diagnostic coding to assess patient acuity and assign appropriate primary diagnoses in collaboration with the Director of Patient Services and physicians
• Participate in bi-weekly and PRN team meetings to review service frequency, duration, and patient progress
• Communicate regularly with PT/OT staff to ensure therapy services align with physician orders
• Serve as the care coordinator for the multidisciplinary team
• Develop individualized care plans with input from patients, families, and healthcare team members
• Document all services provided, including care content and process
• Monitor patient outcomes and update care plans as needed, informing physicians and team members of any changes
• Educate patients and families on disease management, self-care, and prevention strategies
• Maintain clear and timely written and verbal communication to support optimal care delivery
• Perform other related duties as assigned

Qualifications
• Current Registered Nurse (RN) license and registration in New York State
• Graduate of an accredited nursing program (BSN preferred)
• Bilingual (Spanish/English) and UAS certification preferred
• Strong communication, organizational, and clinical assessment skills
