Roles & Responsibilities
• Provide comprehensive diabetes education and case management to patients with diabetes, prediabetes, and gestational diabetes
• Maintain and utilize diabetes registries and RPMS tools to ensure patients receive recommended exams, labs, immunizations, and education
• Coordinate primary and secondary prevention goals through active participation with the consortium diabetes team
• Facilitate provider and staff education on IHS Standards of Care for Adults with Type 2 Diabetes and support implementation
• Lead and support the annual IHS Diabetes Audit and use audit data to improve patient outcomes
• Model and promote the organization’s standards of excellence while delivering patient-centered care
• Respond to inquiries from staff and patients with professionalism and clear communication
Qualifications
• Current, valid RN license in the State of Alaska
• Certification as a Certified Diabetes Educator OR eligibility to apply for certification within two years
• Clinical competency assessment required within three months of hire and every three years
• Approximately 3 years general nursing or dietetic experience preferred, with at least 1 year preferred in diabetic care and management
• Ability to perform diabetic care procedures and safely lift or move up to 50 lbs as required
Education
• Nursing education sufficient for RN licensure (as required to hold a current RN license in Alaska)
Knowledge & Skills
• Strong knowledge of diabetes case management practices, IHS Standards of Care for Adults with Type 2 Diabetes, and diabetes registries
• Experience using RPMS and audit tools to track and improve care
• Skilled in patient education, health promotion, and teaching both one-on-one and in groups
• Excellent oral and written communication and organizational skills
• Cultural competency working with Alaska Native populations and appreciation for local customs
• Comfortable with computer use for documentation, registries, and reporting
