Registered Nurse (Director of Patient Services) in Queens, NY

Director of Patient Services (DPS) – RN L&G Home Care Agency | New York L&G Home Care Agency is seeking an experienced Director of Patient Services (DPS) to lead and oversee clinical and professional home care services. This role is responsible for day-to-day clinical operations, regulatory compliance, staff supervision, and ensuring high-quality, patient-centered care. Key Responsibilities • Direct and oversee daily clinical and professional home care operations • Ensure compliance with all Federal, State (NY), and local regulations • Develop, implement, and maintain clinical policies and procedures • Supervise, train, and evaluate clinical and program staff • Monitor quality, continuity, and safety of patient care • Conduct home visits, staff supervision, and competency evaluations • Collaborate with administration, finance, community partners, and families • Participate in Performance Improvement / Continuous Quality Improvement (PI/CQI) • Ensure 24-hour clinical coverage and appropriate staff-to-patient ratios Qualifications • NYS Licensed Registered Nurse (RN) – active and in good standing • Graduate of an accredited school of nursing (Diploma, BSN, or higher) • 2–4 years clinical patient care experience (acute care preferred) • 2–3 years supervisory or administrative experience in home care (required) • Strong knowledge of NY home care regulations • Excellent leadership, communication, and problem-solving skills • Proficient in Microsoft Word, Excel, and Outlook Work Environment Office, patient homes, and community settings.
