As a Registered Nurse, you will play a crucial role in delivering and directing high-quality nursing care to patients within their homes, as prescribed by their physicians and in alignment with laws and agency protocols. Your coordination with healthcare professionals ensures continuity in patient care through effective communication with physicians, agency staff, patients, families, and community resources.

Key Responsibilities:
• Deliver exceptional clinical services adhering to infection control standards.
• Coordinate care among the patient/client's care team from admission through discharge.
• Conduct comprehensive clinical nursing assessments as per federal/state program requirements.
• Verify patient/client eligibility and medical necessity for services according to agency policies.
• Develop and continuously update individualized care plans in collaboration with other healthcare providers.
• Ensure the care plan frequency and duration align with patient needs and facilitate revisions with physician approval.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• A valid driver’s license, vehicle insurance, and reliable access to transportation.
• Current CPR certification.
• Ability to adapt to diverse home situations, regardless of client background.

Preferred Qualifications:
• 1+ years of RN experience.
• Able to work independently.
• Excellent communication, writing, and organizational skills.

Compensation is based on various factors, including local labor markets, education, experience, and certifications. We offer a comprehensive benefits package, recognition programs, stock purchase equity, and 401k contributions (subject to eligibility). The hourly pay for this role ranges from $28.27 to $50.48 for full-time employment.
