Your Responsibilities Include: • Assessing patient conditions and identifying nursing needs. • Setting realistic health goals for patients. • Monitoring symptoms closely and reporting any reactions to treatments or changes in physical and emotional conditions to the case manager. • Ensuring continuity of care through collaborative work with nursing and other healthcare professionals in a home setting. • Providing educational support and counseling to patients and their families regarding the patient's care plan. You will report to the RN Team Leader and the Director of Nursing. A valid RN license is required; home health experience is a plus, but we welcome new applicants! We value an upbeat attitude, excellent nursing skills, strong communication abilities, and a proactive approach.
